Care Coordinator Job Position Overview:

We are seeking a dedicated and experienced Care Coordinator to join our client’s team in Denver, CO. The Care Coordinator will play a pivotal role in facilitating and coordinating support services for adults with learning difficulties and lifelong conditions. The ideal candidate will have a minimum of 2 years of experience in a similar role, along with previous experience using THERAP or similar care management software.

Care Coordinator Job Key Responsibilities:

  • Client Assessment and Care Planning: Conduct comprehensive assessments of clients’ needs and develop individualized care plans in collaboration with clients, their families, and interdisciplinary teams.
  • Service Coordination: Coordinate and monitor the delivery of support services, ensuring that services are aligned with clients’ goals, preferences, and needs.
  • Communication and Collaboration: Serve as a primary point of contact for clients, families, caregivers, and external service providers. Facilitate effective communication and collaboration among stakeholders to ensure continuity and quality of care.
  • Documentation and Reporting: Maintain accurate and up-to-date records of client assessments, care plans, service delivery, and progress toward goals. Generate reports as needed to track outcomes and compliance with regulatory requirements.
  • Resource Referral and Advocacy: Identify and coordinate access to community resources, support services, and advocacy services to enhance clients’ independence and quality of life.
  • Training and Support: Provide training, guidance, and support to clients, families, and caregivers on topics related to independent living skills, self-care, and behavior management.
  • Quality Assurance: Participate in quality assurance activities, including audits, program evaluations, and performance improvement initiatives to ensure the delivery of high-quality, person-centered care.

 

Qualifications:

  1. Minimum of 2 years of experience in care coordination, case management, or a related role, preferably in a community-based setting.
  2. Previous experience using THERAP or similar care management software is highly preferred.
  3. Strong interpersonal skills with the ability to establish rapport and build effective relationships with clients, families, and interdisciplinary teams.
  4. Excellent organizational skills with the ability to prioritize tasks, manage multiple responsibilities, and meet deadlines.
  5. Knowledge of community resources, support services, and entitlement programs for individuals with disabilities is preferred.
  6. Ability to work independently with minimal supervision and as part of a collaborative team.
  7. A valid driver’s license and reliable transportation are required.
